State Mixed Golf Tournament August 3rd & 4th, 2024 Shotgun starts at 8:30 am

Please read tournament information and regulations carefully before filling out the entry form. $180.00 entry fee per couple.

*With the acceptance of my application I hereby waive, release, and discharge all claims or damages, death, personal injury property damage, which I have or which I may subsequently occur to me as a result of my participation in this event. This release is intended to discharge Supreme Lodge of the World, Loyal Order of Moose, any state Moose association and/or sponsoring Lodge, their respective agent(s), and employee(s) from and against any and all liability arising out of or connected in any way with my participation in this event even though liability may arise out of negligence or carelessness on the part of the person(s) or entity(ies) mentioned above.

**The terms here shall serve as a release and assumption of risk for my heirs, executors, successors, administrators, and assigns in exchange for my participation in this event.

***I acknowledge that I am twenty-one (21) years of age or older, that one person from each couple is an active member of the Moose Order and under no physical disability that would prohibit my/our involvement in this event.

Senior Men 70 and older will play from the Gold/Red Tee Boxes.
